Arriving with your completed Health Information forms will allow us to begin your dental treatment on time. If you have not completed your forms please arrive 15 minutes prior to your scheduled appointment. Your initial exam will last approximately one hour. At this visit we will do a comprehensive oral examination and provide you with an evaluation that will outline your existing dental problems as well as Dr. Lusby’s proposed treatment. In many cases you will also be scheduled with the hygienist for an additional hour to receive a dental cleansing and oral hygiene instructions.
Make your child’s first visit a happy visit.
By putting your child in the proper frame of mind, you can make the first visit to the dentist enjoyable and positive. Tell your child in advance, that someone will look at their teeth. Avoid using words such as "pain" or "hurt" since this may instill the possibility of pain in the child’s thought process. Dr. Lusby will examine your child’s mouth for early signs of decay or other problems. Limiting this first visit to a brief visual exam helps to establish trust before introducing other procedures such as x-rays and cleanings.

For your convenience, you can download our patient registration forms prior to your first visit. You may also fill them out upon arrival but please arrive approximately 15 minutes prior to fill them out.
Financial Information
Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment. For your convenience, we accept Visa, MasterCard, American Express and Discover. We do offer Wells Fargo Financial and Citi Health Card as outside financing sources. We also extend senior citizen discounts as well as discounts for payment in full if treatment is more than $200.
Additionally, we will submit all insurance claims for you. We are a preferred provider for Delta Dental Premier, Diversified Dental Services and Mastercare Dents. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.
